Terms and Conditions

I confirm that I am 18 years of age or older, or that I am legally permitted to order testing with parental or guardian consent where applicable. If parental or guardian consent is required, I confirm that such consent has been obtained. I also confirm that I am the person to be tested, unless I am legally authorized to order testing for another person.

I agree to order my lab test from LabTestsOnDemand according to the instructions provided and to truthfully and accurately answer all requested information. I understand that inaccurate, incomplete, or false information may delay, prevent, or invalidate the ordering, laboratory, reporting, or results process.

I understand that refunds may be available for 10 days from my online order date if I have not already submitted a specimen to the patient service center or laboratory.

• No refunds are available after 30 days.
• No refunds are available for physical products, COVID tests, strep tests, home kits, at-home collection kits, or any collect-at-home kits.
• I understand that there is a $30 cancellation fee that will be deducted from any eligible refund.
• I understand that no changes to laboratory orders or laboratory requisitions can be made once the laboratory requisition or laboratory order has been prepared.

I understand that the laboratory order expires automatically after 90 days and is no longer valid at the lab after that time.

The order may be renewed one time if LabTestsOnDemand, iHLTH, iHLTH.org, or Imperial Health LLC is contacted within 90 days of the original order date.

If the price has increased since my original order, I agree to pay the difference as well as a $30 processing fee to prepare a new laboratory order or requisition.

I understand and agree that laboratory testing purchased through LabTestsOnDemand, iHLTH, iHLTH.org, or Imperial Health LLC does not constitute medical advice.

Laboratory test results are not a diagnosis, treatment, or protocol for any disease or condition. It is recommended that I discuss all results with a qualified healthcare professional.

I also understand that laboratory test results may be inaccurate through no fault of LabTestsOnDemand, iHLTH, iHLTH.org, Imperial Health LLC, the ordering physician, the laboratory, or any related service provider.

I understand and agree that neither LabTestsOnDemand, iHLTH, iHLTH.org, Imperial Health LLC, nor the ordering physician is my medical provider, and that no patient-physician relationship exists or will exist as a result of my laboratory order or laboratory test results.

I agree that my test results are provided for educational and informational purposes only. I agree to follow up with my own physician or qualified healthcare professional if I have any questions about my laboratory results or do not feel well.

I understand that the information on this website is for informational purposes only and is not a substitute for care, medical advice, diagnosis, or treatment provided by a physician or other licensed healthcare professional.

I understand that each test has benefits, risks, limitations, false positives, false negatives, and accuracy limitations that are not guaranteed.

I further understand that test results are best interpreted in the context of my overall clinical evaluation with a qualified healthcare professional.

LabTestsOnDemand, iHLTH, iHLTH.org, Imperial Health LLC, the ordering physician, and their related officers, directors, employees, agents, licensors, suppliers, affiliates, contractors, and third-party information providers make no express or implied warranties, representations, or endorsements regarding the services provided.

LabTestsOnDemand, iHLTH, iHLTH.org, Imperial Health LLC, and the ordering physician shall not be liable for any cost, injury, loss, claim, or damage arising directly or indirectly from transactions conducted through the website, information conveyed on the website, laboratory testing, lab results, delays, errors, omissions, or reliance on information provided through the service.

It is solely my responsibility to evaluate the accuracy, completeness, usefulness, and applicability of all services and information provided.

I agree to indemnify, defend, and hold harmless LabTestsOnDemand, iHLTH, iHLTH.org, Imperial Health LLC, their officers, directors, employees, agents, licensors, suppliers, affiliates, contractors, third-party information providers, and the ordering physician from and against all losses, expenses, damages, claims, liabilities, and costs, including reasonable attorneys' fees, resulting from any violation of these Terms and Conditions by me.

I understand that this agreement shall be construed and enforced under and in accordance with the laws of the State of South Carolina.

Any lawsuit arising under this agreement shall be litigated in a South Carolina state court with jurisdiction for the County of Richland, South Carolina. The parties waive any objections they may have to jurisdiction.

Permission to use this website is at the discretion of LabTestsOnDemand, iHLTH, iHLTH.org, and Imperial Health LLC and may be terminated at any time for any reason.

Any text or structure from this website is solely for the use of ordering laboratory tests through the website. I agree not to copy portions of the website or use the website for illegal purposes.

I agree that LabTestsOnDemand, iHLTH, iHLTH.org, and Imperial Health LLC will not be liable to me or anyone else for any loss or injury caused in whole or in part by procuring, compiling, interpreting, delivering, or reporting information through this website.

I understand that if I choose to be tested for STDs or STIs, I will not be tested for every possible STD or STI. I will only be tested for the specific tests I have selected or purchased.

I also understand that some STDs or STIs will not test positive immediately after infection and may require retesting in 3 months, 6 months, or 1 year in certain situations where a latent period could be involved.

I understand that, although rare, false positive and false negative test results may occur.

I understand that federal, state, and local public health authorities may have regulations requiring certain STDs, STIs, and other infectious diseases to be reported with demographic and personal information.

The laboratory performing my tests and the ordering physician will follow applicable regulations.

I understand that additional confirmation testing may need to be ordered and that such confirmation testing may involve additional costs depending on the confirmation test needed.

I understand that my health insurer will not reimburse me for laboratory tests ordered through LabTestsOnDemand, iHLTH, iHLTH.org, or Imperial Health LLC.

I understand that LabTestsOnDemand, iHLTH, iHLTH.org, and Imperial Health LLC cannot legally provide me with a diagnostic code required by insurers for reimbursement.

I understand that receipts do not include CPT codes.

LabTestsOnDemand, iHLTH, iHLTH.org, and Imperial Health LLC will protect the privacy and security of my personal health information as required by applicable law, including the Health Insurance Portability and Accountability Act of 1996, commonly known as HIPAA.

I understand that every effort will be made to have my results returned within 1–3 business days after specimen collection at the patient service center. However, I understand that results may be delayed, or the laboratory may require a return visit for repeat sample collection.

No result timing is guaranteed.

I understand that I will receive a copy of my laboratory results and that it is my responsibility to share copies of those results with anyone of my choosing, including hospital credentialing departments, my personal physician, school, allied health program, university, college, or any other party.

LabTestsOnDemand, iHLTH, iHLTH.org, and Imperial Health LLC will not send results to anyone else except where required by state health department regulations or other applicable law.

I understand that, by acceptance of these Terms of Service, I certify that I have read this entire document and agree to all terms and conditions. I knowingly and voluntarily use this acceptance as my consent for appropriate laboratory screening and/or HIV/STD testing.

I understand that if I do not hear about my results within 5 business days, it is my responsibility to notify LabTestsOnDemand or the designated support contact for instructions about receiving my results.

I understand that if I do not hear about my results, I cannot assume the test was negative or normal.

I understand that I may not be able to have my laboratory testing performed at a patient service center in the following states due to state regulations:

• New York
• New Jersey
• Rhode Island
• Maryland

Note: State availability should be reviewed and updated by legal/compliance before launch.

By purchasing products or services and agreeing to these Terms and Conditions, I consent to receive promotional communications from LabTestsOnDemand, iHLTH, iHLTH.org, and Imperial Health LLC, including texts, emails, and other forms of marketing messages.

These communications may include updates, offers, discounts, and news about products or services.

I may opt out of these communications at any time by following the unsubscribe instructions provided in emails or by texting "STOP" to the designated number for SMS communications.
